How to transfer the submission of one form to another form

I need to delete a form and transfer the submission of that form to another form. It's exactly the same form,  just an additional button at the bottom that allows the user to upload additional PDFs. You'll find the form under Patient Center, Patient Forms, Well Woman/New Patient (it's the Cornerstone Women's Care History). Thanks! Cindy

You can also check the Import Data app 

Since it has the same form layout, you can get the data from the original form and then use the output CSV data to import it to the new form.

I honestly do not get how to use this Import Data app, it is so difficult that I cannot manage and I am trying to transfer the submissions from 5 different forms into one, to have an overview. Can you please help me? it gives on giving me errors even if the forms are identical, since i cloned them. Thank you in advance!

Please let us know the form name / form ID of all 5 forms from which you want to import the submissions. We will help you in creating the submission excel which you can use to import the data.
