    As I can understand you're trying to upload the PDF file into JotForm; however, this does not seem to be possible at the moment, you cannot import forms in PDF format in the Form Builder.

    However, if you're trying to display the PDF file in an existing form you've created in the Form Builder, then I would suggest you to use the iFrame widget, this way you may simply provide the URL to your file and it will be displayed in the web forms.
