I have just tested your form and I received the email auto-responder, here is a screenshot of it:
Please do note that if you're using the test button in the email auto-responder the email will be sent to the email address you've set up in your account.
May be also that the email address you're testing is in our bounce list, when an email is in our bounce list none of the emails sent from JotForm will be received.
In order to check this, please take a look to the next guide so you can check and if you find the email address there you can remove it: How-to-Remove-Your-Email-Address-from-Bounce-List
If you find and remove the email address from our bounce list, please test again and let us know the results of your test.

Thank You Kevin. The email address I was using was on the bounce list. I removed it and everything is working normally. Thanks again.

If your email address keeps going in the bounce list, you can use your SMTP to send the emails http://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form.
