Upon checking your form, I was able to see that the docusign widget was not being displayed on your form. So, I went ahead and cleared your forms cache and that fixed the issue on my side.
Could you please check and let us know if the form works on your side too?

This problem is still occurring, but now on a different form:

I have cleared the forms cache, but the problem is still happening, i.e. the DocuSign widget is not displaying. What do you suggest next?

Hi,
Have you tried re-adding the widget DocuSign again?
Please try re-adding it again on that form, that should fix the problem.
Also, I would like to suggest these following widgets regarding e-signatures:
2. E-Signature
3. SignPlus
4. Initials

I am reluctant to add the DocuSign widget again as removing the current instance of the widget will result in the loss of data associated with that widget, i.e. signatures already completed successfully.
We prefer to use DocuSign as it is legally recognised in Canada (where we are based) as an electronic signature.
Please advise how to get the current instance of the DocuSign widget on our form working.

Is your form embedded in a website?
If so, could you please share the URL of your website so we can take a look at it?I was able to successfully load the DocuSign widget using your form's direct URL in 3 different browsers:
Chrome
Internet Explorer
Firefox
If your form is embedded in a website there could be a JavaScript conflict that may be causing the widget not to show up. In this case we recommend using the iframe code to embed the form.

The form is not embedded in a website. We have, however, set up a 'webhop' with our DNS service with a more memorable URL for the form in question:
application.bialik.ca
This redirects to the form with which we are having this issue, but, as I said, the form itself is not embedded in a frame or in any other way on a website.

This should not be the case. If you remove the old one, and add a new one with the same label. This would again show the data. If in some off chance it does not work you can always restore it back as it was before of removal of the DocuSign widget: How to View Form Revision History
I was also able to load the widget properly on my jotform, so there is one more thing that I would suggest and that is clearing your browser cache.

The DocuSign widget has been fixed - http://widgets.jotform.com/widget/docusign
We've also made quite a few updates that make it easier to use.
Please give it a try, and let us know.
