confused--do you provide subscription services?

I have been looking at surveygizmo, survey monkey and other survey subscriptions. I thougt by reading your pricing summary you did the same. We dont want folks who visit our website to take the survey. We want the survey conducted in the dance studio...at time of lesson. After the lesson, we would have a device (tablet or pc tyoe kiosk) for instructor rating to be completed. I have created a draft prototype of the evaluation form using jotform.  After student hits submit...we want a new blank survey form ready for next student. Eventually we would like two kiosk-type devices. We also are collecting student signature and require odf view to validate. Does this kiosk envitonment work with your product and where would the results be filed?

If you do not want to embed your form in your website, you could share the direct link to the form instead, this way only those with the link will be able to fill out the form.

How to get the direct link of a form.

Our forms are already mobile device friendly it means that there is nothing to do and your forms will be properly displayed on phone, tablets and desktop devices. 

When your users submit the form they will be redirected to the thank you page; however, you could change this and redirect them to the blank form so another submission can be done.

If you need to capture signatures in your form then you could use one these widgets:
