Any idea what happened/how I can correct the email that I am receiving from our Jot Form Donation Form? The data became scrambled when receiving into Outlook 365, where previously it was not. I have attached screenshots.

I think this is because of its current format, looks like a plain text. I would suggest re-creating your form email to fix the problem.
Delete the current email (https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder).
Create a new one (https://www.jotform.com/help/25-Setting-Up-Email-Notifications).
Please try that and let us know if the issue persists.
