We are not receiving the jotform questionnaire from our website www.epichealth.pro

    We have forgotten the password for our jotfrom account. When we attempt to reset the password it states successfully sent, yet we have not received an email to the email registered with Jotform which james@bodyatwork.co.uk


    Thanks for your help in resolving.

    This is because your email address is in our bounce list, I have already removed it so you can receive the email with reset instructions now. 

    This was also the reason why you were not receiving submissions as you're using the same email address as recipient in your form.
