I use online several online forms to screen patients for trauma, dissociation and other mental health issues. I then print out the results from Jotform and manually enter the data into a scoring sheet. Many of the responses are then divided into various factors, added up and then averaged. A lot of room for error as well as time consuming.
I have attached an example of a scoring sheet.
Can you advise how I might make a spreadsheet template and then automatically export data from form so all calculations are done and I end up with total and averaged scores

It is not possible to create spreadsheet template inisde of JotForm, but you can download your submissions in Excel and then you can customize your Excel document.
