Can my attached form be used with jotform to collect leads?

    I have an insurance business and am looking for additional home and auto insurance leads. I have an assistant that does the work on these requests and she works the best when the attached form is completed and sent over to her. Can this form work in jotform and will it produce a completed pdf that looks like the word doc, but with the information entered in?

    Yes, you can do that in JotForm. You can collect data submission, download submission and export it in excel, csv, or PDF. Please check these related guides:
