How can I setup a table in JotForm?

I need to create the below in Jotform and wasn't sure about the best way to do it? Any help would be greatly appreciated!

Your screenshot looks like a table. If you need something similar, you may use the Input Table field. However, the 3rd and 4th columns may not accommodate the narrow width of the Textboxes within the Input Table. There's a high chance users will type long replies on the Main Duties and Reason for Leaving fields.

If that won't work, you may also consider using Widgets. Some good alternatives are the Configurable List and Infinite List widgets.
