How can I block the same submit to more than one question?

In my Forms, the people have to insert their personal information, like their Id and phone number.

The problem we have is that sometimes just one person answer for many others, and put the same informations. (just the name and the files are from each player.

I'd like to know how to block similar information in more then one question.
