Email Submission after Form Completion

    Good Afternoon,

     

    I have recently signed up for your service to create an online application and possibly future forms for a local EMS Agency. We have been looking for an easy, yet effective way to create and edit forms that we can embed into our site. I found the form creation process was very easy, the only issue I have is the way the form gets submitted via email after for completion. When I did a test resume, a PDF version of the completed form, along with the attachments was sent to my email, which is exactly what we want, except, none of the page titles showed up. I have 13 pages and each one has a different set of questions etc. Is there anyway to make the Headers show up on the completed for that gets emailed out? 

     

    I look forward to hearing back from you. Should this be a simple fix and the forms be able to be sent and organized properly, we will be upgrading our account to a paid membership.

    It is possible to show headers inside of PDF report. To achieve that you will  have to check "Show Headers and Texts" option in the form submissions page. Inside of this guide you can find how to do it:

    Text fields and headers will not be shown inside of email Notification. Only input fields and values of those fields will be included inside of email Notification. However you can customize your email Notification and add headers.
