Is there a way to add a .doc file to my form?

    I am doing a dance registration form.  I want to attach a word document at the end that is our liability waiver and then have a checkbox saying they agree to the liability waiver.

    It's not attaching per se, but you can add a downloadable link to your form that should allow your users to download the .doc file you have. For this, you'd use the Text Field.

    Just ensure that your .doc file is hosted online (e.g. on Google Drive, Dropbox, etc.) as mentioned on the guide above.

    Alternatively, if you wanted to incorporate the .doc file with a checkbox, use the Terms & Conditions Widget then add the URL to the .doc file you have hosted online.
