My client needs to consolidate clothing orders and collect checks/cash from employees. We will not be using PayPal. Need suggestions for order sheet template.
1) there will be 6 clothing options, each in a specific color.
2) the employees gets to choose one item for free and additional items they will pay for, via check or cash to their receptionist.
3) the price for each item needs to either have sales tax added, or have the sales tax included.
4) we already know that one of the clothing items is on backorder, so that notation will need to be added.
5) we want to save the order form for future use. Does it "expire" at any time?
6) I will be adding photos of each garment. Do you require jpg?
What template do you recommend?

I do not think that we have a template for your specific case, but you may check available templates with the 'order' keyword, or just create it from the scratch.
1) there will be 6 clothing options, each in a specific color.
You may try a Purchase Order tool. It does not require online payment and support multiple products selection.
2) the employees gets to choose one item for free and additional items they will pay for, via check or cash to their receptionist.
As a workaround, it may be possible to use a coupons feature to offer one product for free.

Tax settings are also available under the purchase order tool.

You may include additional notes to the product name, or add a text element to the form.

No, created forms do not expire.
6) I will be adding photos of each garment. Do you require jpg?
Jpg, png and gif images should be fine.

Unfortunately, those are the only options for coupons. Can you tell us the instance when does the free one put into effect? Is there a minimum required items or is this one always readily available for someone who makes a single purchase?

Do you have access to the entire thread? This is a one-time order. We will set an expiration date for employees to order. There are six wearable options...three men's and three ladies. Each has a different price. The employee can pick one item at no charge, paid by their employer. They have the option to order more, at their own expense. So, ideally the free item would not show up in the total due.

Can you create multiple Fixed Amount coupon codes, one for each product?
You can also create a products and set price to 0 (free). You can create 6 products and set price to 0 and 6 additional product where you will set prices, so that users can order more.
As alternative you can create your products using basic field and the use form calculation widget or conditional logic to perform calculation.

You can also pass calculation to payment field.

Where do I find the ID of my form?
I am using the Purchase Order payment setting. The only drop down garment size options says "T-Shirt". I am not offering a t-shirt. How do I change this to Select a Size? Or, remove it entirely?

I noticed you have not specified your product option prices. You need to enable the special pricing per option settting. Like this:
Dont forget to hit the green "SAVE OPTION" button

I loaded your form from its direct link (http://www.jotform.us/form/71855797184170) and the drop-down shows "Select Size:" on my end.
Created a clone version of your form and I see the Product Option label is "Select size:".
If this is not what is showing on your end, I suggest you try to test your form using a different mode of your browser or clear your browser's data.

Here is the link to my revised version. I was told to use the Purchase Order feature. I was hoping the Excel worksheet would be separated by item and size. Instead the Excel worksheet is just a listing of orders, one order per line item. Please advise.

Unfortunately, we do not currently have an option to split the product data into separate columns. It may be possible to manually split the data in Excel, please take a look at the next thread.

At the bottom of the form, we would like a separation between the FREE items and the for PURCHASE items. I did have a dotted line, now I have changed it to "Purchase Additional Items Below". How do we remove the word FREE? I understand the form needs a dollar amount, but it is not needed on this line. Please advise.

If I'm understanding you correctly, you are referring to this (please see image below)
in the builder page or this, (please see image below)
when the form is viewed.
To remove that, you will need the CSS codes below injected to your form — How-to-Inject-Custom-CSS-Codes

I have checked the CSS codes of your form from a clone version and found that there is a missing open curly bracket in the CSS codes and there is also a missing line (font-weight: bold;)
Please update the codes and save it after.
