can't add survey to facebook page

i have created a survey and want to add it to a specific facebook event that is part of a page. i have followed the publish steps but do not see anything added to the FB page. i don't see an option to embed it to the specific event either.

it works via the contact tab. thanks. what i need to do is have the survey be part of an event that is on our page, not a tab of the main page. is it possible to embed it directly on an event of the page?

Its not possible to embed your form to on a Facebook event page. However, you should be able to post the link in the "Tickets" section.

Copy your forms URL:

Where to Find My Form URL?

And then paste it.
