We are testing a new form and when we receive a submission, the form is missing almost all information even though i filled out the form entirely.Page URL:

The notification email template fails to update itself after you made the changes in the form builder. The best way to fix this would be to re-create the notification email.
You should delete the existing template (https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder).
You should create a new one after (https://www.jotform.com/help/25-Setting-Up-Email-Notifications).
