My form sent me the email but the field entries are missing

We are testing a new form and when we receive a submission, the form is missing almost all information even though i filled out the form entirely. 

The notification email template fails to update itself after you made the changes in the form builder. The best way to fix this would be to re-create the notification email. 

You should delete the existing template (https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder). 

You should create a new one after (https://www.jotform.com/help/25-Setting-Up-Email-Notifications).
