How to send forms out to a team?

Hi Is there a way of sending a form out to a team and the system keeping track of who signs up.  We are a dance school and enter dances into competitions.  Our teams can have from 10 - 25 KIDS and we are looking at ways of tracking entries to events.

You can send your form as a link in your email address and send it to your team.

You can track entries by setting up email notification, you would be able to receive emails when you receive a response from your web forms.
