Can you tell me why my account was suspended?

    Hi, Can you tell me why my account was suspended? I have a medical practice and need access to these forms with important information. Thanks!

    Your account got suspended because the "Credit Card Authorization" form is collecting Credit Card information using Basic fields.


    As per our terms, collecting credit card information is not allowed here in JotForm.

    We need to delete the guilty fields so we can re-activate your account. Let us know if that is okay with you.

    I have now reactivated your account and deleted the said offending fields to prevent it from getting Suspended again.

    you deleted all my forms and information that had not been transferred. I need this back. The credit card form was only to be deleted.

    I have reviewed your account and I can see my colleague just deleted the credit card fields from the form

    The other forms are still on your account, there is a total of 5 active forms, I also checked your account activity logs and I can see nothing else was deleted, submissions nor forms, you may also check this guide and review your account activity logs: 

    I you have noticed missing submissions/forms, please provide us more details about it so we can further investigate on our end.
