    I have a question.

Is there a way I can use a form as a tool for employees in my building?

For example, if a maintenance person is walking around and notices damage to the building. He/she could use their phone to fill out a simple form, submit the form containing details of the damage. Maybe some pictures and a list of tasks needed to repair the damage, which would then integrate with a task managing software like Asana.

It's possible that there would be multiple types of forms that an employee would need to fill out for different types of incidences. It would be great If the forms existed in an app that could be opened from the phone's homescreen.

    Any help would be appreciated

These are all possible with Jotform.

For listing specifics, you can use our Long Text Entry field.

For the taking pictures, you can use our file upload field (https://www.jotform.com/help/332-How-to-create-upload-forms).

    For Asana integration, https://www.jotform.com/help/470-Create-tasks-in-Asana-with-JotForm-submissions.

You can create multiple forms with Jotform. But since there is a limit, you may need to upgrade the account. The free plan is limited to 5 forms only. Please check our pricing page to compare the details:

You can add this widget in your form to prompt users to add the form link to their homepage: https://widgets.jotform.com/widget/add_to_home_screen. This only works with IOS devices, so you may consider asking people to just save the link on this phone notes or bookmarks to access it later.

I hope that helps. Let us know if you need any further assistance.
