Is the basic package incude 100 forms every month

is the basic package include 100 form Everymonth?

And do I need to have a I pad at the location. and the client can fill it out from there home aswell.

Please note that the plans have  different limits, with the free one you will be able to receive up to 100 submissions, have 5 active forms at the same time in your account, receive 3 payments. The limits reset the first day of each month in EST time. 

Also, the forms can be filled from any device with internet access, we do recommend to use either Google Chrome or Firefox while working on JotForm forms.
