Is there a known method for collecting an e-signature with a Jotform and populating the e-signature onto the document created in google slides (using autocrat)?

    Thank you,

    Spencer

I don't think that's not possible. You'll end up getting a link of the signature image instead of the actual image.

You may consider using our PDF Editor instead https://www.jotform.com/products/pdf-editor/. You can have the images display on the PDF from the upload fields of your form.

    Thank you.
The problem with the PDF editor is I cannot match the exact format of the document that I need to produce.

You can customize your PDF report, please refer to the guide below:

How to Customize PDF Submissions Report.

If it's okay for you, could you please show us the document that you want to replicate so we can check if it's possible or not?

    Thank you!

Let me try to recreate your form using custom CSS.

I'll get back to you as soon as my sample form is complete.

    Thank you.

To make sure that I understand, I can have a regular jotform where a user can answer questions and those answers will populate this pdf.

    Is that correct?

    Thank you

I have placed the image of your PDF file on top of the form and use CSS to place the fields.

Can you please test the  https://form.jotform.com/90230908808963 form and let us know if this fits your requirements?

We can create a Form and edit the PDF submission. However, I'm not sure we can recreate your PDF fields completely.
