How can I make it so that my form does not have additional options for names and addresses? I want it to only appear IF the client needs those additio

You can set up conditions to hide specific fields in your form. I have prepared a simple demonstration for you here.

What I have done is I have hidden the fields that I want to appear when the user wants which are the email and address fields.

Next, I have set up two different conditions to make those fields show themselves if the according option is selected in the dropdown box. You can set up your own conditions by going into Settings->Conditions in your My Forms page.

This is for the address field to show up:

This is for the email field to show up:

This is just a simple demonstration and you can do much more complicated and interactive forms using conditions. We have a guide that explains how to use conditions in detail which you can find here.

This is great thank you. I had a quick question. Do you know of a better way if we have about 5 applicants that are going to have more than 5 owners, how can I make it so it can keep popping up new name and new address fields for every person?

Do you already have form fields created so we could take a look? Generally, you can ask users how many options they need to fill and show number of fields based on that answer.

Alternatively, you can check if previous set of fields name+address is filled and then to show next set of name+address gradually.

We will be glad to assist if you face difficulties with this.

    Hi Victoria,

Here is the link to the form. It will be if they have more than one administrator or owner and more than one venue address.

I'm not the most tech savy and I'm better with the general process of jotform so any help is appreciated.

Before to proceed with conditional logic, I would like to suggest using a widget. If you need to only collect information on administrators or owners, you could use Configurable List widget. It allows to allow users adding sets of fields dynamically by pressing a + button:

    Hi Victoria,

The widget was a great suggestion. I am having trouble seeing how the widget can lead the applicant to a new section where they enter each additions name, email and address?

I am sorry, but I am not sure if I understand the task correctly. Basically, you can allow users adding sections where they enter each additions name, email and address using Configurable List.

I see several fields for addresses, names etc on your form https://form.jotform.com/90106726213145. Could you please explain a bit more how you would like them to work? We will wait for your reply.

Sorry Victoria I know it was a little bit all over the place but I think we are essentially saying the same thing. Let me try using the configurable list again and have you take a look to see if I'm doing it correctly.

I think I'm going to use the Matrix Dynamique widget instead. Is there a way I can change the of the headers so it doesn't say head 1 and head 2?

It seems you have already updated the header values in the widget.
