If you want the users to upload an image file in a jotform that will be embedded in one of your webpages, simply add the File Upload field to that form. Please follow this guide: https://www.jotform.com/help/332-How-to-Create-File-Upload-Forms 

If you want to redirect the users after submission to a specific webpage where you want them to upload files, you can use the Redirect to external link option:
