Is it possible to create a category form/quiz?

I am looking to make an form/quiz/assessment that based on the answers to questions will show different end pages.  Like a personality quiz for example.  "since you answers the questions this way, you are an Extrovert"

Is this possible within JotForm, if so, how?

You can create multi-pages using "page break" and hide/show pages based on the user's answer. Or, you can use the "section collapse" tool to hide/show content based on the user's answer as well.

    Hello,

Unfortunately, these two items are not what I was looking for.

Currently when a participant fills out a form, they are taken to a thank you page.  Is it possible to take them to different thank you pages based on their answers?

Yes, please go to Settings > Conditions > Change Thank You Page and set the condition. Then in the Do part select "Display a custom message on submission" and customize the Thank You page. You can also set the redirect URL.
