Can I create my own form?

I found some similar form with my requirement, not 100%meets, can I get a new form from you or can I change some contents on exist form according to my requirement?

also how can I get the applications after people submit the forms

When the form is submitted, the submissions are automatically stored in your JotForm account. You may check the submissions of the form by logging into your JotForm account.
