How to create a work order form?

I am trying to create a couple forms to help my employees in the field easily check off and give quantities of items needing to be repaired. This is for landscape and irrigation scenarios. they walk with ipads and it would be nice to have standard checklists they can make notes and give quantities. I am feeling a little lost trying to build something like that. The data from the field walks will need to be able to be translated into communication documents and or formal work order for pricing so we will need to be able to get the data from the forms
