    HELLO,

I have already created my forms on Adobe Acrobat Pro.

Because I do not like how most of the companies who are form creators take up wasteful space needlessly on the web page due to its default formatting and there are no templates for my needs, I find it more efficient yet less cumbersome to create one myself on Adobe.

The one I created has all the elements I wanted but it is not as aesthetically attractive when seen online and also not mobile to actually be able to fill it out.  Therefore, if I send it to you can will beautification and mobile readiness can you make it as follows:

    1)   comprehensive as my original,                                                                          2)   aesthetically attractive and professional                                                            3)   web, mobile friendly not only to view but to fill out                                              4)   iframe embeddable for my site as well as to facebook page

If you cannot do any of the above, I will glamorize it a bit myself.

However, can you host it or else I will continue to have the issue with dropbox as the settings are not for embedding but linking.  Also need it for mobile resolution ready.

I will highly appreciate your.  I am just one person in my business.

I will hereby only send you the short version of the form.  If you can do what I require I will send the full application.

I LOOK FORWARD TO YOUR RESPONSE.

    Sincerely,

    Rossie@ZealCache.com

p/s:  I see you do not accept the pdf version so sending only jpg

    Hi Rossie,

You can actually import your PDF Form to JotForm. Here's a guide: https://www.jotform.com/help/548-How-Can-I-Import-My-Own-PDF-to-JotForm

This will generate a web form where you can fill all the fields and once you're done you simply have to click the Review and Submit button and all the entered data will be previewed on your PDF form.

Example https://form.jotform.com/91968250591971 (click Start Filling button on the top right corner to view and fill the web form).

Or if you prefer creating a web form out of your PDF form version, you may start creating from scratch then customize it in the Advanced Designer or inject custom CSS codes to get your desired layout.

We will also try to recreate your sample and provide you a link. We'll be back later today.
