How I convert a pdf to a survey form to be used on a Squarespace site?

    Hey I am still so confused about this.  Can you look at this site, https://craig-goodwill-wr6c.squarespace.com/forms.  I used your pdf converter and it didn't parse it in the correct format with the correct fields and looked really weird.  I then went through the whole thing and formatted it, took forever.  Then embedded it on the Squarespace site.  It shows the original PDF like I wanted in the first place but says "start filling" on the top right, when I click on that it goes to what I redid.  I am confused.  Also, where is this info going?  Please help!

    Hello!

    I see you have successfully embedded a PDF form to SquareSpace and it seems to work as it should work. PDF Converter, which you have used to set up this form, is used to collect data from fillable PDFs, but it can not capture it from the PDF directly. So, it converts fields into an online form. This is the one you see after clicking 'Start Filling'. The data users enter will go to your JotForm account, you can review submissions following this guide: How-to-View-Form-Submissions

    Can you provide some more information on how you would like the form to work. Maybe you only need an online form and not a fillable PDF?

    Thank you.  So I just really wanted the form to look like the pdf.  When you click start filling it goes to the version I had to redo because all the fields were off.  Should I be doing this a different way?  Ideally it would just convert that pdf into useable form fields.  Does that make sense?  I have one more form with a table you can view at https://craig-goodwill-wr6c.squarespace.com/nutrition-forms.  How can I easily convert this with the table and blacked out squares.  It keeps erroring.  Please help ASAP...THANK YOU!

    I also need to transfer this form over to a new owner as I am working on this site for a client.  He wants it under his name and he will upgrade to the bronze package.  How do I transfer the form and create the new one under his account?  Thanks again!

    When you click start filling it goes to the version I had to redo because all the fields were off. — Do you mean that the PDF fields you see before clicking the "Start Filling" button doesn't match the fields where you are redirected?

    I've compared both and fields are there.

    Please note that if you embed the PDF online, users will still need to download the PDF file and open it using Adobe Acrobat, fill then submit the form. Those information will be sent to your online form's submission page which is an equivalent to your PDF. 

    As for your other concern, please create a new thread requesting which form you want to transfer and to which account.

    Those fields only match after I went through the whole thing and made them match.  I also had to reformat and ask questions differently because the convertor couldn't read 90% of it.  It changed the types of fields scattered the text and basically had to be redone in a way that is unsatisfactory in design.  As for my other question regarding the second form I posted, the PDF, that can be found as a link on  https://craig-goodwill-wr6c.squarespace.com/nutrition-forms just so you could see, that was never answered.  It just errors when trying to upload to your site. I found the submissions section, thank you.  It needs to be transferred so I will create a new thread for the account transfer.  Please help ASAP.  Thanks again!

    The form in the link you provided cannot be imported as there are incompatible elements, for example, the table at the beginning labeled "Please complete this section" does not match an element that can be interpreted on the PDF editor, kindly note that if there are incompatible the import will fail.

    As far as I understand the task, you would like to create a fillable PDF form so it looked exactly the same as the one you shared when users are filling it out upon hitting 'Start Filling'. I'm afraid this will not be possible to do online (to fill a PDF form). But, you can use our different tool, PDF creator, to create a downloadable PDF form: https://www.jotform.com/fillable-pdf-form-creator/ 

    Using this tool you can create a fillable PDF form for users to download and submit using PDF readers software like Adobe or Foxit reader. In this case fields will not be converted to different view, users will see the form exactly as you created while loading via PDF reader.
