Suspended account

Hi,

My account is shown to be suspended as I cant login into my account and yet I think im not collecting any password from the responder. Yet, I have made the form for a important event that still happen right now and I need my account to be back as soon as possible and know the reason why my account is suspended.

Your account is suspended because you are collecting passport numbers without agreement from your users, which is against our Terms of Use. 

If you want to collect passport numbers, you need to add the Terms & Conditions and Signature widgets to your form, for users to confirm/allow/agree that you are collecting the information for a purpose.

We can only lift the suspension of your account if you agree with our terms and allow us to delete the offending field.

I i have recently get reply from your team and told to edited my form an add the terms & conditions and signature widgets to my form. but then if my account is suspended hwo do I log in to my account and edit my own form ? I hope you reply asap.

You have added the terms & condition widget however the actual terms and condition are not mentioned. You are using this widget to get form users' consent about sharing their confidential information.

Currently, there are no terms added to your form it only redirects to the default URL. You can either use your own webpage URL where the terms are mentioned or use a free text field where you can mention the terms. 

Users will read, understand and then agree to condition. 

Here is how to add the text field where you can mention your terms and conditions. You can then add the terms and condition widget below this.
