I need my form to be used multiple times from the one device

Forms created in JotForm are designed to be submitted multiple times on any device.

Do you wish to reload the form to have empty fields every after a submission?

You can do that by setting the Direct URL of your form (Where-to-Find-My-Form-URL) as the Redirect to External Link of your form on the Thank-You page settings.
