Do you have Squarespace integration?

Hi, 

I'm looking for a form plugin to add to my squarespace website. 

I'd like artists to be able to upload a submission with their artwork (jpg files) online but need help understanding how to do that. Is your program integrated with squarespace? Does it work across mobile and web? Can I create a table where they can fill out information?
