Login Failed-Suspended

    I have tried numerous times to reset my password but no matter what I do, I keep getting the Login Failed-Suspended Message. Will you please help me?

    Your account is suspended because your New Hire Enrollment Form is violating our Terms of Use. The form is used to collect Social Security Number, which can be against our Terms. 

    You can only collect the Social Security Number is you also add Terms & Conditions and Signature widgets to the form for users to allow/agree/confirm that you are collecting the information for a purpose.

    Let us know if you agree with our Terms and we will delete the offending field and reactivate the account.

    I agree with those terms. Please delete the social security number field and reactivate the account.

    I delated the Social Security Number field. Your account InterCool is now active. If you want to collect the Social Security Number, please add the mentioned widgets to your form.
