What is the best way to layout this form?

    Hi Guys,

    i have created a form, but struggling to find an eloquent way to let people completing the form - what best widget shall I use for:

    1. The Clubs can select up to 10-15 competitions / teams - we'd need the following info for EACH team - Age Category, Team Captain' name and surname, their phone and email, and Club name.

    2. I'tried to use the condition on the form depending on what form of payment - direct transfer or credit card people choose - but I dont think I got this right?

    3. Can I use a widget that would calculate the total payable depending on the number of competitions entered (fields completed) and shows the amount due?

    Can you please share more information how you would like your form to work?

    You may use the configurable list widget to create multiple fields.

    To clarify, do you want to create an option to allow your form respondents to pay through direct transfer or credit card?

    You may hide the payment field so that your form users will only submit the form.

    3. Can I use a widget that would calculate the total payable depending on the number of competitions entered (fields completed) and shows the amount due?

    You may assign values to a field and use the form calculation widget to calculate.
