My IP address is blocked

    My IP address is blocked . I can't access jotforms.  Please email and help me with this situation

    Your IP is shown as banned because your account is suspended. The form "New customer order form" was found collecting credit card info via plain form fields and also collecting Social security & driver license numbers without mandatory widgets: 

    1. Credit card info:

    Due to our Terms and our PCI compliance it is not allowed to collect credit card info in forms.

    If you want to collect payments from your customers, you can integrated any of our authorized payment processors into your form - https://www.jotform.com/integrations/category/payment-processing

    2. Social security & driver license numbers:

    In order to collect these numbers, you will need to add the mandatory widgets mentioned below in your form:

     - Terms & Conditionshttps://www.jotform.com/widgets/terms-and-condition 

     - Signaturehttps://widgets.jotform.com/widget/signature 

    In order to activate your account we will need to delete the credit card fields so that you can add these widgets. Please review these and confirm.

    As confirmed, I have deleted these credit card fields and activated your account. Please add the above widgets into the form to avoid getting flagged by our phishing tool again.

    Hi . I keep getting blocked . I tried to use widget but I don't want to
    charge credit card or take payment thru jot form . This jotforms is used
    internal for taking orders as the information will come to me via jotforms
    then I will input to a portal for DirecTV . It was translated in Spanish .
    Please help me . Mike Molina. Maybe you can let
    me know what widget to use . As I'm a novice at this.

    Your account was again suspended because you are again collecting credit card numbers via plain form fields.

    As already explained in my earlier response, you cannot collect credit card numbers via forms. This is in violation of our Terms and our PCI compliance.

    We will need to delete these fields to activate your account.

    I have already mentioned above, the widgets that you need to use in the form. Here is the list:

     - Terms & Conditions - https://www.jotform.com/widgets/terms-and-condition  

     - Signature - https://widgets.jotform.com/widget/signature

    ok i will delete the fields ,,my assiostant put field in when translating
    ,, i will seek another way ,,, but please un block my account as i will be
    using jotforms just to collect leads ,, no credit cards ,, , i will try to
    see if im allowed to use stripe or square widgets .... please let me know
    any helpful info you can give me

    Thank you for your cooperation. I have now deleted the offending fields from your form and activated your account. Please check and let us know if you need any further help.

    Please refrain from creating such forms or adding any guilty fields to the forms to avoid any further suspensions on your account.

    Your IP address is whitelisted. Kindly try to log in to your account again and let us know if the issue persists.

    so i am able to log in but all my jotforms are disabled ,,, does this mean
    i have to redo all my jotforms ,, i cant click links or open in new tab
    says form is disabled on all forms

    The status of your forms is disabled, to enable the forms, please follow this guide: How-to-Enable-Disable-a-Form
