Once the form is live, who pays for the assessment?

Once the form is live, who pays for the assessment?  Is it the user that completes and submits the form and do they receive immediate feedback from their form submission? 

I am not sure if I have understood your question correctly. Please note that if you have added any payment tool in form, you need to integrate it with your merchant account. When user submits form, they will make the payment and it will be sent directly to your merchant account.
