I need to create a daily report form but the data needs to be editable

Each day we have our drivers fill out a report. we collect all kinds of information. One piece of that information is letting them know when they have returns they need to pick up. It has data like, where the return is, how many pieces there are, the tracking number, etc. Our dirvers need this information to know when and where to go in order to get the returns. Each day our office enters this information for them. I need to create a table at the top of the form that will show this information and allow them to report on it. Most of the data tables widgets dont work becasue they put all of this information into one cell when its exported. 

I would like to use the spreadsheet to form widget, which would allow us to pre-popoulate fields based on a uniqe identifier. However, i would have to build the table using short text entry boxes. 

Any suggestions on how we could accomplish this? Ive attached a snapshot of the google sheet each driver fills out each day.

Forgot to mention, if you create the form in the Classic layout, and you would like to position the fields to form a table layout, our forms allow CSS injection. If you hire a CSS developer, he should not have any issues styling the form. Unfortunately, we don't have such a service, but you can find and hire someone at Upwork.com.
