PDF submission forms missing fields/data

When i get a submission and download the pdf i can see where the client initialed the field but the field text is missing and some of the contract information such as the price listed etc are not showing up on the pdf. can someone help me with this?

You just need to click the +New PDF button.

After that, you need to update the email settings and enable the PDF attachment:

I was able to get the new pdf doc to show everything but i dont see the email settings options to change
