How can I have the body of a paragraph show up in the submitted form?

    Hi,


    I am trying to create a patient pre-registration form for our medical practice. Part of this document includes consent form and other forms with signatures. However whenever I test submit the document the signatures appear but the text of the consent forms does not appear, making the form useless.


    If you could please help with this process I would really appreciate it.

    Hi,

    There are 2 options you can try to make these work.

    1. If you were referring to the form Submission page, you just need to enable the option Show Headers and Text.

    Ex:

    2. If you meant instead in the Email Content (Notification Email), the Paragraph field is not included by default.

    So you will need to add it manually (normally by copy/paste method).

    Alternatively, you may use the Fill in the Blank field instead of the Paragraph field. The new Fill in the Blank field will always show up and available both in Submission page and Email Content of the form.

    The Fill in the Blank field is almost similar to Paragraph/TEXT field but it has input property. You can just add one input/blank field and do not set it as *required so the user does not need to answer the blank field.

    Thank you for the help. That problem has been fixed. However now that that problem has been fixed another has popped up.
    When I receive the document as a PDF attached to the notification email, the signatures are missing. See screenshot below. Additionally when I go to download the PDF form from the browser the signatures are moved and not associated with the correct consent forms.
