COVID-19 forms

    Hello,

    We are a music contracting company. I will need a COVID-19 waiver for both the musicians that work for us, as well as one for the clients that we serve. Can you help me with these 2 different versions? My company is Fifth Avenue Entertainment.

    Also - how do I publish the form to my WordPress website? Do I just provide a URL of a blank page?

    Hello,
    I am using your COVID-19 Liability Release Waiver. The Date, Title and 1st paragraph do not show up in my email; this is the one that states, “The World Health Organization has declared the novel Coronavirus …….”, and includes my company name. However, if I print a PDF from the website, I do see the entire form with all of the components. Is this the intended way to use the form?

    Hi - That is intended for the email content. You need to manually add the Text/Paragraph field to the email content.
