Form was suspended for phishing?

So I was reading about why you suspended my form - we are using the form as an online application which gathers social security numbers and driver's license. How should we move forward with this process if we are no longer able to gather that info for job applications?

You're correct. Your account got suspended because of the SSN form field. You can still ask for SSN and Driver's License but you need to add these widgets:

Also, you need to update the email address registered to the account using the company email.

If that is okay with you, please let us know so we can activate your account back to good status and you can add the widgets and update the email address registered to the account.

The email registered is our company email - unfortunately, we all have Gmail accounts. Is that going to make it so I can't build this form?
