Send Street Address, City, and Zip Code only to Google Doc

    Hello,

I just signed up and I'm excited to be using JotForm as our new Contact Form for new jobs/customers.

We are using our JotForm contact form to sync up with our Quotient Estimating software and with our Google Drive documents and it seems to be working great.

I am having an issue with the Address Element. I only want and need the Street Address, City, and Zip Code on our form. I've made only those options visible on the form, leaving 'Street Address 2', 'State', and 'Country' non visible.

When I fill out the form using pre-populated data from my device, it looks good. But then when the data goes to my google doc or the Estimating software, I'm getting a duplicate on the Street Address. It's as if the pre-populated data fills both the Street Address Line 1 and Line 2. So it looks like this:

14373 S Peacock Midge Dr, 14373 S Peacock Midge Dr, Bluffdale, UT, 84065

The double street address is then causing problems with our google map link and also not allowing some of our forms to be created. How would you recommend working around this? I thought about just using the 'Short Text' Form Element but I'd have to do 3 separate elements for the address (Street, City, Zip) and that looks weird. I don't see any other Form Elements that are set up like the Address that has multiple boxes?

I also noticed in the Address Advanced Setting under Field Details this information. Can the #input_7_addr_line2 be deleted? I didn't see how I could edit that?:

    UNIQUE NAME

    { }

The field can be used to pre-populate fields from URL and to pass data to another form automatically. It is also important that there should be no spaces in between.

    FIELD IDs

    #input_7_addr_line1, #input_7_addr_line2, #input_7_city, #input_7_state, #input_7_postal

This id can be used to style your forms using CSS codes.

And then the other question I have is the Time Stamp. Can that be formatted differently? Right now it reads like this:

    2021-02-01 22:19:32

    I'd prefer this instead:

    February 1, 2021 22:19:32

    or 02-01-2021

    Thank you for any help.

Welcome to JotForm! How are you passing data to your Google Drive? The form does not appear to be integrated with any third party integration:

We have a way to stripe headers from postal addresses in Email Alerts: https://www.jotform.com/help/92-how-to-strip-headers-from-postal-addresses-in-email-alerts

However, it cannot be done through integrations.

If you need to prepopulate the address fields, these are the URL parameters to use:
