Hello,
I am creating a new FORM, and tracking submissions. I noticed that when I edit the form, all of the sudden on the right hand side it is asking me to "re-upload the PDF form" ? I need to know how I can get this back and making sure that downloading the submissions will be ok and the PDF will not be affected.

Apologies for the inconvenience.
Perhaps the PDF file you upload got corrupted. Thus, asking you to re-upload.
If you still have the PDF on file, please try re-uploading it and let us know if the issue persists.
I also cleared your form cache from our end as a precaution.

I do not recall ever changing the settings to remove this PDF - If I no longer have it avaialble, how do I recreate it? The form is published, but we haven't made it public to receive any submissions. We'd like to have it all set up this week for that. So its time-sensitive.
How can I re-upload or re-creat the fillable PDF for this particular form?

Sorry about that. I'm afraid we cannot recover corrupted PDFs. Perhaps there's still a copy on your local machine that you used when you uploaded PDF the first time.
You can also try restoring your form to its previous version just to check if it makes any difference.
