How to create a form with an option to pick what form needing to fill out?

Do you want to create a form, that will let your customer choose which form they want to fill in, and then they will be redirected to this form?

If this is what you are looking for, you can do this by redirecting users to a different page when they submit your form. You can use a different URL for each answer based on a condition, to direct the users to different forms.

If you were referring to the classic and card layout, unfortunately it is not possible to let the user choose on the form in which layout they want to view the form.
