Patient can't fill the form online ..

    Hello there,

    Thank you for reaching out to Jotform Support! I just cloned your "Madison Apothecary COVID-19 Vaccine Screening Form" into my own account, and was able to leave a couple of submissions without issue. Can you please confirm that the form referenced above is the same form you are having issues with?

    If so, can you please let us know whether this is only occurring on an embedded version of this form, or if it occurs on your freestanding form as well?

    Hello there ,

    I hear from customers that this happens when you fill the form on desktop , no problem with phone app .

    I don't understand where the issue comes from .

    Thank you for the quick response! Can you please request some additional details from one of the customers who is reporting this issue? Are they unable to access the form at all, or is there a specific point in the form at which they are getting stuck?

    Additionally, can you please explain how you are distributing your form to your clients? Are you sending out the direct link to your form via email, or is this form embedded on your webpage?

    Hi ,

    My understanding is that when they go to questions 7,8,9 , a pop up window comes up and don't let them continue .

    Hello Pencho,

    Thank you for the follow-up! I just tried filling out my cloned version of your form once more, and was able to do so without issue. I just noticed that you are using a redirect for your Thank you page that is sending your users to https://forms.ny.gov/s3/vaccine

    Is the issue that is being reporting occurring on your form, or the form linked above?

    Lastly, if this issue is affecting your form, are we authorized to leave a test submission to try this out? Is the form embedded on your website, or are you sending out a link to your patients?

    I'm unable to login into jotform account. It shows "There was an error. Please try again later." Also my form is disabled. https://form.jotform.com/211586060160043

    Hello Andrew ,

    Jotform is connected to our website , once they they click for vaccinations goes to the submission form they have to fill , when they start filling out the form on question 7 or 8 pops up window for allergies and don't disappear.

    I hear this from everyone using desktop . Then I have to tell them to do it on the phone !

    Thank you again for the follow-up! I was getting ready to run a few tests here when I looked back over this thread and realized that you still have not authorized us to leave submissions for this form. Do I have your permission to leave a couple of submissions on each fo these forms to test this behavior out?

    It seems the input table description is blocking the users to be able to complete the input table.

    You may use custom CSS to fix the issue.
