Portrait Session Contract form with e-signature

Hi,

I am looking for a basic form to make a portrait session contract for photography clients to sign with scope of work etc...and an electronic signature.  Wondering if you can point me in the right direction I am not finding anything adaptable yet as I search...

Also I am wondering if this will work to insert into a "code block" on my Squarespace website for my Squarespace form???
