Unable to see the form in the template gallery

    Hello,

    I'm looking for the Medicaid Intake Form from the link below but I'm not able to locate it in the templates.

    Can you please let me know how to find this form? I just signed up for your service, hoping to have access using this form.

    Hello Lloyd,

    Thanks for reaching out to Jotform Support. If the owner of the form did not share the form as template, it won't be visible on the Form Templates page. Please reach out to the form owner to see if they can share it as template.

    Also, it's not possible to clone the form because it's enabled for HIPAA features. If you don't have a contact to the form owner, the best avenue is to manually create the form. You can check this guide on how to create a form.

    Thank you for the responding.

    I'm trying to recreate the form but I don't know how to create the single choice with a text field that appears based on the selection.

    I want a box to appear when someone selects NO so they can input more information. I don't want that box visible if the person selects YES. It only appears when the selection is NO. Can you let me know how to do this?
