    Olá viktor944,

    Tenha em atenção que quando você editar o seu e-mail alerta modelo torna-se sujo. Depois que todas as alterações feitas em seu formulário não serão refletindo no modelo de e-mail.

    Eu sugiro que você exclua os alertas de e-mail existentes e adicionar um novo. Um novo alerta por email sempre exibirá todos os campos do formulário nos e-mails de submissão. O guia a seguir devem ajudá-lo:

    Adicionar novo alerta de e-mail:  https://www.jotform.com/help/25-Setting-Up-Email-Notifications 

    Eu também verificado os campos do formulário e eu achei que você mudou o "nome" do que para um texto muito ling. Existe alguma razão específica para isso? Por favor verifique a imagem abaixo:

    Hello friend, I tried to delete and add again but it did not work.
    The problem was in the very long fields as you said, had done it because he preferred centralized. I put them on the left and it was good.
