My school is creating a gift card fundraiser. We need to create an order form with gift cards from different sponsors and offer multiple quantities and prices for each. How can I create that form that the table will allow us to clearly see how to fulfill orders?
So for instance. We might have ten locations offering gift cards. Each of those can be ordered in $10, $25, $50, $100, $200, and $300 amounts. And our clients might want to order 2- $10, 1- $50, and 1- $200 from one location.

Hello Tonya,
Thanks for reaching out Jotform. To start with, you might consider checking the ready-made Form Templates we have that you can use or get ideas for creating your own form. You can see the Form Templates that offer/sell gift cards from this link.
Creating the form that you wanted is easy, let me walk you through a sample that I made on the screencasts below:
- On the orange navigation toolbar, go to the Build tab.
- Click on the Add Form Element.
- Look for the Dropdown Element and add it to the form, this is the field where we will add the location.
- Then on the Add Form Element again, add the Product List Element to the form this is where we will add the gift cards as products of the form.
- Add the gift card products to the Product List Properties on the right side of the form.
Part one:
Part two:
Are you going to charge the user after they add their purchase? If yes, what payment option would you prefer? Jotform has lots of payment integrations that you can add to your form, check out this link to see all of them.
If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?
Give it a try and let us know if you have any other questions.

Okay, so I was hoping I didn't have to have 6 different options for each type of card. We have a LOT of different stores with cards in different amounts. This is what I had started with so far. Am I doing this wrong? https://www.jotform.com/build/222774777345165

Hello Tonya,
Thanks for getting back to us. I can see that you were able to set it up properly. If you want to collect amounts with no payment integration, you can set up Purchase Order Element. This guide would show you how to do that.
Give it a try and let us know if you have any other questions.
