Having trouble in uploading PDF document as a form.

    Hello,

    I am having trouble when uploading a PDF consent form that was provided to me from the manufacture of a machine that I will be using. There are a series of Yes/No questions on this form, but when I upload the consent form to Jot form it is now allowing the client to choose yes or no to each question.


    Can you please help me to have this form uploaded properly?

    Thanks for reaching out to Jotform Support. I understand that you would like to use your own PDF document to create a form. Can you please share the form URL so we can look into it?

    Please note that if you uploaded a PDF document, the details on the document will also be included. If you don't want the form to show those Yes/No questions, you will need to delete those Yes/No questions on the form.

    You can check out this guide for more details about importing your own PDF document with Smart PDF Form.

    Thank you for your response. The consent form that was given to me to use was from Hydrafacial, and I have it in a PDF saved. There are a questions that clients need to answer prior to treatment that are YES/NO as seen on the form. When I upload this form the Jotform, the YN goes all the way to the bottom and is not alined with each question for the client to select.
    Can you help me to have the consent form look the way the PDF attached does?

    Hi Nicole,

    Could you please share with us a copy of the PDF? Or could you share with us the link to the uploaded PDF form so we can also check it on our end?

    Hi Nicole,

    Thanks for getting back to us. Unfortunately, PDFs cannot be uploaded to the support forum. But you can upload the document to an online storage service like Google Drive and provide us the public link.

    Once we hear back from you, we'll be able check the file.

    Hi Nicole,

    Thanks for getting back to us. When I tested your PDF, the same thing happened for me. What you can do is manually edit your form to look like your PDF by removing unnecessary information and separating it into a new paragraph element, as well as including a Single Choice or Multiple Choice element in your form and mapping the fields to your PDF. Let me show you how:

    1. In Form Builder, click on Add Form Element in the upper-left corner to open Form Elements.
    2. In Form Elements, search and select Single Choice or Multiple Choice under Basic.
    3. Place the blue box in the YES and NO boxes in your PDF after changing the texts. This must be done for each of the questions.

    You can clone the form I made and change some of the fields to match your PDF. You can check out this guide on How to Clone an Existing Form From a URL.

    Give it a try and let us know if you any more help.

     I rebuilt the form using the advice you gave me below.I think it worked out well, thank you for the suggestions.
     Now the only thing I am having touble with is changing all of the forms to have my New Logo. Can you tell me how I can change this on each of the forms?I still see my old black, white and grey one appearing.

    Hi Nicole,

    Thanks for reaching out to Jotform Support. I checked your form and do not see any logo added. It seems that you are referring to the image added here:

    You can delete the image above and add a new one or add a logo instead as seen just above the image.

    Here's a shortened version of the steps:


    1. Sign in to JotForm.

    2. Create a new form.

    3. Choose "Upload PDF" option.

    4. Upload your consent form.

    5. Review and edit form fields.

    6. Adjust field types to Yes/No (radio buttons or checkboxes).

    7. Customize the form's appearance.

    8. Save and preview the form.

    9. Publish and share the form.


    Following these steps should help you upload your PDF consent form to JotForm with functioning Yes/No questions.
