My emailed form is different to my actual form

    I have used a template to create my form and when i use it to fill in it reads as it should, but when it is emailed to me it reads as per the template I used, the details that were filled in are correct on the email, but the text created on jotform is as per the original template.  Can you help please?

    Unfortunately, the output in the email will not look exactly the same as the template or layout of the form.

    Many thanks for your reply, but I am not sure I explained myself well enough.  The image on the left is the form prior to completing, once completed it is emailed to myself and appears as the image on the right.  The only information that comes through is the data filled in, the 'questions' aren't the same.

    Images, texts and headers aren't automatically added in the notification email. If you want to include these fields in the notifier, you will have to manually add it by adding rows or columns.

    Then type the texts and add the URL of the images using the image tool.

    As for the field labels or questions in the form, I would suggest re-creating the whole settings. Delete the current notifier (https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder) and re-create it https://www.jotform.com/help/25-Setting-Up-Email-Notifications
